Operation Sight Volunteer Surgeon Registration

Operation Sight is the ASCRS Foundation's U.S.-based charitable cataract surgery program. It involves a nationwide network of volunteer organizations and ASCRS-member surgeons who provide care to those unable to access or afford surgery on their own. Operation Sight strives to provide accessible care right in the patient’s home community. Once registered as an Operation Sight volunteer surgeon, you will be contacted should a pre-qualified needy patient in your local area be identified. You are free to accept or defer the case based upon your schedule and availability.

Payment of the $250 per case Operation Sight stipend requires the volunteer practice/surgeon to submit a Form W-9. Upload the completed W-9 in the form below or email to Evelyn Morales at emorales@ascrs.org.
